WorkGoing on leave
If an employee is going on a leave of absence, here are some steps to consider:
Ask any questions about requesting a paid or unpaid leave and initiate paperwork by calling the HR Support Center at 1-877-564-4772.
Employees must make sure their manager is aware that they’ll be going on a leave of absence. If the leave is due to Short-Term Disability (STD), they’ll need to enroll in STD. If they anticipate they’ll be out for more than seven consecutive calendar days, they can call the Disability Claim Telephone Reporting Service at 1-800-260-2170 and they’ll be assigned a Disability Case Manager.
Employees should make sure their name, address and any other personal information is correct in People at Liberty in case we need to communicate during the process.
If an employee is enrolled in an HSA or DCFSA, their contributions will cease while on leave. For more information on the extent of these changes, employees can call the Liberty Benefits Center at 1-800-758-4460.
Through the Work Life Solutions (EAP) Plan, employees get access to resources, immediate advice over the phone and referrals to online and face-to-face counseling. Employees receive up to 10 free visits per family member, per issue, each year. They can into the Portal to view the Work Life Solutions website and phone number.
