FamilyNotifying us of a dependent death
It can be very difficult to know what steps to take after losing a loved one. If an employee loses a spouse, domestic partner or child who was covered on any health or insurance plans through Liberty Mutual or was a beneficiary of an employee’s life insurance, below is a checklist to help keep a terrible event from becoming even more painful:
The HR Support Center will work with the employee and their manager to explain the bereavement leave policy and prepare for bereavement leave.
On that call, the employee will need to select the “Report a Death” prompt. A condolence letter will be sent with information about any applicable benefit changes, spouse or dependent life insurance payouts and Ayco Survivor Support.
Employees have 30 days from the date of death to make changes to their benefits. If an employee doesn’t make changes within 30 days, they’ll have to wait until the next Annual Enrollment period or until they have another qualified status change. They can make eligible changes to health and income protection plans by visiting BenefitConnect or calling the Liberty Benefits Center at 1-800-758-4460.
If applicable, review and update beneficiary designations for retirement, life and AD&D insurance benefits. Log into the Portal for a full list of direct links to the various beneficiary designations.
If an employee’s spouse has passed away, they might want to make sure their tax withholding and exemptions are up-to-date. Visit People at Liberty to update tax elections.
Employees must make sure your important legal documents are up-to-date so they have a plan in place to provide for loved ones. They have access to self-help resources for financial documents as well as a no-cost 30 minute legal consultation through the Work Life Solutions (EAP) Plan.
